Providers
Providers are essential to our community, working with people to help them reach their goals. Working in the field of developmental disabilities is a very rewarding experience. The Ohio Department of Developmental Disabilities certifies providers in Ohio.
We contract with the Mid-East Ohio Council of Governments (MEORC) for provider support services. They assist with the initial and renewal provider certification process, compliance reviews, and billing questions. Major Unusual Incidents/Unusual Incidents (MUI/UI) Information and Forms:
UI/MUI Reporting
If you are concerned that an MUI has occurred, contact the Service and Support Administration Office at 740-373-5147, Monday – Friday, 8 am to 4:00 pm. If you are reporting outside regular business hours, call 740-516-5970.
A copy of the UI/MUI report should be emailed to the designated SSA and to incidents@wcbdd.org by next business day at 3:00 p.m.
MUI/UI Form This form is to be used when something unusual happens in a person’s life.
UI/MUI Reviews
Annually, providers are required to review their major unusual incidents for trends and patterns.
MUI Analysis Form is completed by January 31st and submitted to the county board by February 28th each year. All reports must be emailed to analysis@wcbdd.org.

MUI Annual Review – Independent
Monthly, providers are required to review their unusual incidents for trends and patterns. An Unusual Incident Log Review Form must be completed even if there are no Unusual Incident Reports that month. UI logs should be emailed to analysis@wcbdd.org.

In the event you suspect a person with a developmental disability is being abused or neglected, first ensure the safety of the individual and then immediately report to the WCBDD by calling the 24/7 on-call at (740) 516-5970.
Reports may also be reported to the Ohio Department of Developmental Disabilities Abuse and Neglect Hotline by calling 1-800-617-6733 or by reporting online.
